I try to keep it at a minimum to get the shot. But don’t care to bump it to absurd level if it helps. Better to have a noisy picture than no picture at all.

Neck pain= bad use of gear. If you have a strap and let the gear dangle from your neck, it will cause pain. A shoulder strap is the only decent way. Especially with two cameras.
As for hurting from weight it can quickly become too much if you bring lights and all. I often carry between 20 and 30kgs of gear on small shoots and sometimes even more. If you were to transport all that on your back all the time, it would hurt. So find ways to transport only the necessary gear, use rolling carts, etc. It should never hurt otherwise it becomes a problem soon enough.
Cristal Meth? Buy nice paper and a good printer (and postprocess carefully without obvious HDR ;)
